Prerequisite

Initial :

Students complete American Heart Association Core Instructor Class.

Students must have a current American Heart Association Health Care Provider Status. ( 2006 updates )

Must have a current BLS Instructor manual (80-1011) or ( 80-1016 )

( must be ordered and present in class )

Renewal :

Instructor must be current in all 2005 AHA Guidelines

Instructor must have a current Instructor Card ( NO GRACE PERIOD)

Instructor must show proof of teaching 4 classes in last 2 years

1. Please go on line to www.onlineaha.org for your online Healthcare Provider CPR

HCP online renewal Class --- $17.50

After you receive your certificate . . .

2. Please call 480-998-5193 to schedule you skill Check off - Cost for Skill Check off $15.00

Step B

1. CORE PROGRAM MUST BE COMPLETED PRIOR TO CLASS DATE
NO EXCEPTIONS.

( Applies to all classes after November 1 2006)

Instructor Applicants can pick up Core materials at the Mesa office, or we can ship the materials to you for additional $5.00.

Fax this certificate to our office & bring this certificate with you the day of the instructor class. (MUST HAVE)

Order your BLS Health Care Provider Instructor Manual (80-1011)
or BLS Health Care Provider Instructor Package ( 80-1016 )

**You only need the Manual if your company already has the package including all DVD’s for Teaching. Make sure you bring your Instructor manual with you the day of class.

*The American Heart Association strongly promotes knowledge and proficiency on BLS, ACLS and PALS and has developed instructional material for this purpose. Use of these materials in an educational course does not represent course sponsorship by the American Heart Association. Any fees charged for such a course, except for a portion of the fees needed for AHA course material, do not represent income to the Association
